“We are extremely excited about this partnership with El Camino Hospital,” says Mike Plaia, PharmaPoint CEO. “Everything this organization does is based on the belief that improved technology leads to improved patient care. The addition of an onsite outpatient pharmacy and incorporation of XchangePoint’s acute care platform will provide integrated workflow management of bedside delivery of discharge medications, bedside consultation and pharmacy-led medication reconciliation to enhance the dynamic healthcare services El Camino Hospital already delivers.”
XchangePoint is a SaaS platform designed for pharmacy and care team members to drive medication optimization for at-risk patients in the acute, post-acute and ambulatory care settings. By taking a targeted and proactive approach to medication management, the system will enable El Camino Hospital to close gaps in care resulting in improved patient outcomes, reduced hospital readmissions and increased patient satisfaction.
“Our clinicians and care teams continuously look for ways to improve the patient’s experience and enhance healthcare services,” says Cheryl Reinking, Chief Nursing Officer, El Camino Hospital. “Selecting a forward-thinking and innovative partner like PharmaPoint affords us the opportunity to offer new services to our patients.
