Strategies to prevent med reconciliation breakdowns
Dec 19, 2013: Pennsylvania hospitals experienced more than 500 breakdowns in the medication reconciliation process between November 2011 and November 2012, according to the December issue of the Pennsylvania Patient Safety Advisory.
