The partnership will target Next Level PRIME members plus their spouses and dependents that have been diagnosed with diabetes, hypertension or high cholesterol. XchangePoint’s MedTransition platform will proactively help identify patients that are nonadherent or have potential interactions with their drug treatment plan.

PRIME members also have access to around the clock telemedical and virtual visits along with health and wellness coaching and behavioral health and emotional wellness counseling. Additionally, with PRIME, members have access to a full scope of high-quality care, including blood draws, lab testing, vaccines/immunizations and health screenings.
